Register With The Practice

To register with the surgery the patient must live within the Bridgend Town and surrounding area (a map is available at reception). The patient will be required to fill in a questionnaire (listing current problems, relevant medication and previous medical history). The patient must then make an appointment with the nurse for a new patient check.

How Do I.... Change My Details?

It is extremely important that you notify us of any change of details so that we can keep your medical records up to date. We require any change of details to be notified in writing - ask at reception and they can give you a form to complete.
